JOB SUMMARY:

The Visiting Nurse Association of Erie County, a part of the LECOM Institute for Successful Aging, is accepting applications for a Full Time Hospice RN Case Manager to plan, coordinate and provide hospice care in accordance with the patient’s plan of care.

MINIMUM QUALIFICATIONS:
Education and experience equivalent to:

  • Must be a graduate from an accredited School of Nursing.
  • Must be licensed in the State as a Registered Nurse.
  • One year of RN experience. Hospice experience preferred.
  • Must have excellent assessment skills and problem solving skills.
  • Should be skillful in organization and in the principles of time management and have knowledge of case management processes.
  • Must be a licensed driver with an insured automobile in good working order.
  • The ability to communicate well, both verbally and in writing.

TUITION REIMBURSEMENT:

The RN and LPN Tuition Reimbursement Program was created to assist LECOM Health employees by offsetting a portion of their educational expenses. Registered Nurses or Licensed Practical Nurses may be eligible for tuition reimbursement up to a maximum of $22,000.*
