Visiting Nurse Association

JOB SUMMARY:
The Hospice Administrative Assistant is responsible for the office management and administrative support of the Hospice Department of the LECOM Visiting Nurse Association. This position will also provide back up and weekend rotational coverage for the Intake Department.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

· Will report to any facility within the LECOM Institute for Successful Aging as assigned;

· Assists with Hospice and Administrative Assistant functions as directed;

· Greets people as they call into the Agency, answers general information questions and delivers messages;

· Types memos, correspondence, agendas, reports as required;

· Scans and electronically files patient admission information;

· Photocopying as required;

· Monitors faxes, forwards to the appropriate people and puts pertinent information received into the patient chart;

· Develops and implements a tracking system to assure organization and timely submission of signed documentation into the patient chart;

· Coordinates with liaisons to deliver orders and plans of care for physician signature to offices and hospitals;

· Maintains communication with the Hospice Administrator for continuous follow-up on outstanding orders;

· Ensure that a Bereavement Assessment is done within 5 days of admission as part of the comprehensive assessment;

· Provide referrals to community resources and professional services when deemed necessary;

· Helping to manage the volunteer program for the agency and working with Social Work and other designees to maintain and recruit volunteers;

· When asked help provide volunteers with assignments. If the assignment is with a patient, the Volunteer Coordinator reviews the Volunteer Plan of Care with the Volunteer before they make a patient visit;

· Attend IDT Meetings and prep IDT sheets;

· Print Med sheets/Med Reconciliations for IDT to review and file;

· Perform Compliance and Monitoring for CTIs-Recert/HIS/POC/F2F;

· Perform In-house Referral Management of equipment and initial calls and referrals;

· Print weekly notes to be uploaded into the SNFs chart files and IDT Notes;

· Act regularly as an Intake Associate and assist in the coordination of patient care;

· Receives all intake information for paraprofessional cases and enters the appropriate information in the computer;

· Completes intake forms and routes them appropriately for admission approval;

· Receives referral information from Liaison team and passes the referral information to the appropriate staff as soon as possible;

· Enter referrals the day they are received into Referral Manager;

· Updates referral board prior to A.M. and P.M. meetings;

· Places calls to physicians to confirm who will sign orders (document in chart);

· Places calls to patients to discuss admission and be alerted if patient has an appointment or refuses services;

· Enter all information relevant for the team to complete admission;

· Update referral with any changes prior to admission;

· Confirm F2F date for physician referrals and enter into case communication note;

· Enter ROC orders, scheduling note, and ROC case communication note and send to appropriate team member;

· Email intake team census numbers and referral log in the A.M. and P.M.;

· Check insurance for authorization at time of referral. Enter a case communication note into McKesson;

· Obtain authorization according to insurance requirements within time constraints. Enter insurance communication notes with visit authorizations obtained and send to team members;

· Monitor authorizations for completion and need for continued authorization requests;

· Follow up authorization process within 24 hours of receiving notification that forms are complete;

· Maintain an accurate, timely and daily authorization process and corresponding log to track progress;

· Must be able to take direction and receive assignments from the Hospice Administrator and other management personnel from time to time; and

· Accept other duties needed/assigned for the Institution’s needs.

MINIMUM QUALIFICATIONS:
Education and experience equivalent to:

  • High school education
  • Minimum of six months office experience
  • Must possess typing and other clerical skills
